Every Dallas ISD Health Savings Account held on the DataPath Summit app transfers to the ZyneraHealth Benefits app in a single bulk transfer. Nobody files an individual transfer form. Here is the whole sequence and what it asks of you.
Three phases. The only one that asks anything of you is the first.

Who acts, when, and what changes for you at each stop.
A short online form authorizes the trustee‑to‑trustee transfer of your Summit HSA balance to HealthcareBank, a division of Bell Bank — the custodian behind the ZyneraHealth Benefits app. It takes about three minutes. Your consent is what puts your account in the bulk transfer.
hsaconsent.zynerahealth.com/dallasOnly cash transfers. If any part of your Summit HSA is invested, sell it and turn off automatic investing before the blackout begins.
Your Summit balance is frozen. You cannot spend, withdraw or file against the funds that are moving, and cards drawing on that balance stop working. This is the part members actually feel, so plan for it.
The prior custodian closes out the accounts and sends a single payment for the whole group, together with a file naming every member and their exact amount. Nothing is mailed to you and nothing is yours to chase.
We load the file, post each balance to the right account, and check every figure against what DataPath sent before anything is released.
Balances appear in the ZyneraHealth Benefits app and your card works against them. Nothing further is required of you.
Three things, all before the blackout starts. Everything after that is ours.
Online, about three minutes. Choose the option to email yourself a copy when you submit.
Move any invested balance to cash in the Summit app and switch off auto‑invest, so your whole balance is ready to move on time.
Know you have care or a bill coming during the freeze? Use your Summit card or file that reimbursement before it starts. Otherwise keep another way to pay and reimburse yourself after December 1.
The freeze is narrower than it sounds.
A trustee‑to‑trustee transfer is not a distribution and is not taxable. It does not count against your annual contribution limit and it is not reported as income.
Anything already in your ZyneraHealth Benefits account stays available throughout, along with payroll contributions that post during the window.
Care you receive during the blackout remains eligible — you just cannot pay for it from the money in transit. Keep your receipts and reimburse yourself once funds land on December 1.
Transferring is voluntary.

Be aware of the two dates that follow: ZyneraHealth stops supporting HSAs on the Summit app on January 1, 2027, and Summit portal access ends May 31, 2027.
