Authorize your HSA transfer

Dallas ISD is moving every Health Savings Account from the DataPath Summit app to the ZyneraHealth Benefits app in a single bulk transfer. Your authorization is what puts your account in it. The form takes about three minutes and does not ask for your Social Security number.

Two things to do before November 16. If any part of your HSA is invested, sell those investments and turn off automatic investing — only cash can transfer. And if you have care scheduled or a bill you were planning to pay from your HSA during the blackout, use your card or file that reimbursement first.

Transferring is voluntary. Do not submit this form, and contact DataPath Financial Services directly at fiops@dpath.com to make your own arrangements.

Two dates to know either way: ZyneraHealth stops supporting HSAs on the Summit app on January 1, 2027, and Summit portal access ends May 31, 2027.
